Lakeview Family Health Team
The Lakeview FHT is composed of an inter-disciplinary team of physicians and other health professionals who work collaboratively to provide patient-centered and comprehensive primary care to residents in the towns of Colborne, Brighton, Quinte West, and surrounding areas.
The FHT is composed of physicians, nurses, nurse practitioners, a physician assistant, dietician, and social worker, as well as administrative support staff. There are three sites including Quinte West (Trenton), Colborne, and Brighton. The Brighton location is in the Brighton Health Services Building at the following address:

Health Care Connect
Health Care Connect refers Ontarians who don't have a physician to a family health care provider who may be accepting new patients. Unattached patients can call or go online to register with the program or call Service Ontario at 1-866-531-3161. To successfully register, unattached patients must have a valid OHIP card and complete a health questionnaire to determine their need for family health care services. Priority is given to individuals who have greater health needs.
